10. Research Assistants, Teaching Assistants, Lecturers, and Instructors should bring a written permission document from the universities they work at.
11. Students with the Perfusionist Competency Document or those without this document should show a document, which was obtained during the periods determined by the conditions of graduation, indicating that they work as perfusionists (for students in the Evening Education Program without Thesis)
